School Nurse's Office

Crossett Public Schools announces the opening of the nurse’s office for the 2008-2009 school term. The office is located at 301 W. 9th, Crossett, Arkansas. Call 364-8003 for more information. There will also be a nurse located at the Crossett Elementary School, Donna Robertson, RN.

All kindergarten students who have not pre-registered must be registered through the nurse’s office. Requirements for kindergarten registration are a legal birth certificate, social security card, a current immunization record, a dental exam, and a physical examination. Forms for the dental and physical exams will be available at the nurse’s office and Administration Office.

NEW STUDENTS transferring into the district must also register with the school nurse before going to the school to enroll. Requirements for new student registration are a legal birth certificate, social security card, and current immunization records.
